Implication for health policy/practice/research/medical education: This case illustrates the clinical course, diagnostic challenge and management difficulties associated with adult-onset severe nephrotic syndrome secondary to diffuse podocytopathy, with needs for bilateral nephrectomy as a last resource of therapeutic measure.
